Improving diabetes outcomes with a Personalised Care approach

According to Diabetes UK, there are 3.7 million people in the UK diagnosed with diabetes, a further 4.6 million currently living with diabetes and an additional 12.3 million people at risk of developing type 2 diabetes. This has resulted in diabetes care within the UK currently costing the NHS £1.5 million per hour. In total, this comprises 10% of the NHS’s total annual budget. This is because, not only does the immediate care of diabetes patients need to be catered for through prolonged treatment and medication, but also the long-term effects and complications resulting from diabetes, such as emergency hospital admissions, early patient retirement, amputations, loss of sight and an overall reduced quality of life.

However, the key to reducing the impact of long-term health conditions such as diabetes, to both patients and the wider healthcare system, is not just through more effective treatment or technology, but something more fundamental – changing the relationship between patients and healthcare professionals, so that patients feel empowered and motivated to take better control of their diabetes themselves. To enable this change in the patient/professional relationship, we worked with Erewash Clinical Commissioning Group (CCG) to run a Health Coaching programme where we re-educated and shifted the mindset, habits and behaviours of health and care professionals, enabling them to have more powerful conversations with their patients and lead to better outcomes.
